Material failure of a quality-controlled latex condom is uncommon. The much larger gap between "perfect use" and "typical use" comes from how people deploy them in the moment. Most condom failures trace back to human error rather than to a tear in the latex.

Real-world failure modes worth naming:

  • Late application. Penetration starts before the condom goes on. Pre-ejaculate carries enough HIV virus and bacterial pathogen load to transmit, so a few minutes of unprotected contact still counts.
